Step 1.
Click the button below to go to our online registration intake forms. (Fill out each section of the forms, and be sure to hit the “submit” button at the end.) *You can also always print & sign the forms and bring them with you or fax signed forms to 620-792-7052.

Step 2.
Once the form is submitted, we will reach out to you to set up your appointment time.
– You will need to provide your Photo ID ( Drivers License.)
– Provide your Insurance Card (if you have one.)
– Provide Proof of income (last years tax statement or current paycheck.)
– Fees or copay is expected at time of consultation.
**The Center for Counseling & Consultation will not refuse services due to inability to pay.
We will file with your insurance company first and then can offer you a reduced fee based on your income. We are happy to work out a payment plan that is affordable for you.**

A little about us...
The Center, established in 1968 by the local Mental Health Association in conjunction with area county commissioners, is a Community Mental Health Center. Organized under Kansas Law, it is governed by a board composed of citizens from each of the four participating counties and includes consumer and family member representation. The Center is staffed by licensed mental health professionals serving residents of Barton, Rice, Stafford, and Pawnee Kansas Counties.

The Center serves a wide range of individuals, from children to senior citizens, with a variety of concerns. From school, family, marital, substance use, and work-related difficulties ranging from mild to severe. We address difficulties facing your everyday life.
